New Patient Information

We’re excited to welcome you to Logan Vision Clinic. If you’re visiting us for the first time, here’s what you can do to prepare:

  • Arrive 10–15 minutes early

  • Bring your vision or medical insurance card(s)

  • Bring any current eyewear (glasses, sunglasses, or contact lenses)

  • Bring a list of medications, if applicable

  • Let us know if you have a specific concern or request

Online Forms

Save time by filling out your forms before your visit. We recommend completing these at least one day before your appointment.

If you’re unable to complete the forms online, no problem. Arrive a few minutes early, and we’ll have paper copies ready.

Insurance & Coverage

We accept many vision and medical insurance plans and are expanding our network regularly. If you’re not sure whether we’re in-network with your plan, visit our Insurance Page or call our office directly. We’ll help you check your benefits and explain what’s covered.
